For good and valuable consideration, the receipt of which is hereby acknowledged that I consent to the use of photography submitted of myself and/or my place of work (dealership), and the use of these forms of media taken of me, the absolute irrevocable right and unrestricted permission to publish, reproduce, edit, exhibit, project, display and/or copyright my image/ likeness and/or name in all forms of media, including but not limited to print, digital, electronic, internet, broadcast (hereinafter media) singularly or in conjunction with other photographs and/or recordings for, but not limited to, advertising, marketing, promoting, publicizing, commercial or other business purposes (hereinafter marketing). I waive any right to royalties or other compensation arising from or related to the use of the image or product. I waive any right that I may have to inspect and approve the finished product that may be used or to which it may be applied now or in the future, whether that use is known or unknown to me. I also give my full and complete permission, without limitation for Auto/Mate to take, record, publish, display or obtain testimonials or other statements (hereinafter use) from me in any marketing media form. I further consent to the reproduction and/or authorization by Auto/Mate to reproduce and use said forms of media, for use in all domestic and foreign markets. Further, I understand that others, with or without the consent of Auto/Mate may use and/or reproduce such forms of media. I understand and agree that the statements I made may be used, reproduced, re-used, altered, modified, edited or changed with or without identifying me and my affiliation.
I hereby certify that my testimonial or endorsement of the products and services are true and accurate and reflect my honest belief in the quality of the products and services that I am endorsing. I have no affiliation or connection with, their agents, representative or assignees, or any person or entity connected with. I hereby release Auto/Mate and any of its associated or affiliated companies, their directors, officers, agents, employees, customers, and appointed advertising agencies, their directors, officers, agents, employees from all claims of every kind on account of such use. I further relinquish all rights to use any of the above-referenced images or audio recordings for my own purposes. I understand that Auto/Mate reserves the right to utilize whichever testimonials it chooses, and that is under no obligation to utilize my testimonial. I have read and understood this agreement and I am over the age of 18 OR I am the parent or guardian of the minor named above. I have the legal right to consent to and do consent to the terms and conditions of this testimonial marketing media release.
